IMPORTANT:
- After filling up the form you will take the print of Application Form , Rules and Admit Card each on A4 size paper
- Paste your recent passport size color photograph
on Application Form and Admit Card
- Write DD details on Application Form and put your
signature and your parent/guardian signature on Application Form, Rules
and Admit Card
- You will attach a SINGLE DD of Rs.1,000/- (Rs.2,000/- in case student applying from outside India) in favour of 'BANSAL CLASSES PVT. LTD.' payable at 'KOTA, RAJASTHAN' alongwith the form (SINGLE DD IS MUST, MORE THEN ONE DDs WON'T BE ACCEPTED)
- Also attach attested photocopy of your required Mark Sheet
- Put all above in envelop and send it to "BANSAL CLASSES PVT. LTD., A-10, GAURAV TOWER, ROAD NO. 1, IPIA, KOTA - 324005 (RAJASTHAN)"
- Please check the bank code at the bottom of DD (specialy in case of State Bank of India and Syndicate Bank), it must be of KOTA, RAJASTHAN

NOTE:After receiving your form with DD we will
send you our brochure which will be containing a blank Application Form and Admit Card etc.
Since by this time you have already sent us the Application Form and Admit Card, you need
not to send it again.
- If you missed something to fill in the form please make no correction manually in printed form. Fill a new form if you missed something to fill in the form.
